HBOT After Surgery: What the Evidence Says About Recovery

If you're facing surgery — or recovering from it — you may wonder whether hyperbaric oxygen could help you heal. The honest answer depends entirely on what kind of surgical situation you mean.

The established part

For specific post-surgical problems, HBOT is a recognized treatment, not a speculative one. Compromised skin grafts and flaps — where transplanted or repositioned tissue is at risk because its blood supply is marginal — are on the UHMS-recognized list, supported by the same oxygen-and-angiogenesis biology described elsewhere in this series (UHMS Indications Manual, 15th ed.). So is delayed radiation injury, a real problem for patients who've had radiation therapy and later need surgery or develop non-healing tissue damage in the treated area. In these situations, HBOT has a legitimate, evidence-backed role.

The promising-but-unproven part

Beyond those recognized uses, there's growing interest in HBOT as general post-operative support — reducing swelling, supporting healing, easing recovery after elective procedures. A 2025 systematic review of HBOT for musculoskeletal and recovery applications noted more consistent functional-recovery benefits in some postoperative and nerve-injury settings, while emphasizing that much of the evidence rests on small studies (Musculoskeletal review, MDPI, 2025). That's the "Emerging" label in action: mechanistically reasonable, some supportive data, but not the settled, strong-evidence standard that graft/flap or radiation-injury use enjoys.

What this means for you

If your surgeon raises HBOT for a recognized problem — a threatened flap, a radiation-damaged surgical site — that's well-founded medicine. If HBOT is being offered as a general "heal faster after your cosmetic procedure" add-on, treat it as a reasonable but unproven option, ask what evidence supports it for your specific case, and make the decision with your surgeon. Anyone promising dramatically faster healing or guaranteed results for routine surgery is outrunning the evidence.
