Few therapies attract as wide a range of claims as hyperbaric oxygen. As someone trying to make a real decision, the most valuable thing you can have isn't a list of benefits — it's a map of where the evidence is strong, where it's promising, and where it's mostly marketing.
The strong-evidence core: recognized indications
The clearest dividing line is the list of uses recognized by the Undersea and Hyperbaric Medical Society (UHMS), the field's main scientific body. As of the 15th edition of its indications manual, there are 15 UHMS-approved indications, with avascular necrosis (bone death from lost blood supply) added as the newest (UHMS / ACEP Undersea & Hyperbaric Medicine Section, 2024).
Those recognized indications include emergency conditions usually treated in hospitals — decompression sickness, carbon monoxide poisoning, gas embolism, gas gangrene and other severe infections — alongside conditions treated in outpatient centers: compromised skin grafts and flaps, certain non-healing problem wounds including qualifying diabetic foot ulcers, delayed radiation injury to soft tissue and bone, refractory osteomyelitis, and certain thermal burns (UHMS Hyperbaric Medicine Indications Manual, 15th ed.; UCLA Health hyperbaric indications). These are the uses insurers in North America generally recognize, and they rest on the strongest evidence.
The emerging tier: promising but not settled
Beyond the approved list is a growing body of human research into conditions like post-concussion syndrome, stroke recovery, long COVID, fibromyalgia, and cognitive aging. Some of this work is genuinely encouraging and comes from randomized trials — but the studies are often small, and some findings await replication. "Emerging" is the honest label: worth taking seriously, not yet settled.
The hype tier: where skepticism belongs
Then there's the marketing frontier — HBOT sold for anti-aging, "detox," general wellness, and athletic edge. Some of these are areas of legitimate active research; others have little behind them. The U.S. Food and Drug Administration has publicly cautioned that HBOT is not proven for many conditions for which it's promoted, and that unproven use can leave patients without effective care (FDA Consumer Update on HBOT).
How to use this map
A trustworthy provider will tell you which tier your situation falls in — and won't dress up an unproven use as established fact. Physicians can lawfully order HBOT off-label when their judgment supports it; that's normal medicine. But you deserve to know which category you're in before you commit time or money.
