Hyperbaric oxygen has been delivered safely in clinical settings for decades. But "in clinical settings" is doing real work in that sentence — a pressurized, oxygen-rich environment demands engineering, screening, and protocols. Here's an honest look at the risks and how good programs manage them.
The common, minor risks
The most frequent side effect is ear or sinus barotrauma — pressure discomfort as the chamber pressurizes, similar to an airplane descent. Trained staff coach you through equalizing techniques, and it usually resolves easily. Temporary changes in near vision can occur with longer treatment courses and typically reverse after treatment ends. Some people feel fatigued after sessions.
The rarer, serious risks
Less commonly, pressure and oxygen exposure carry risks that make screening essential: oxygen toxicity affecting the lungs or, rarely, causing seizures; and, importantly, fire. A pressurized, oxygen-rich space is flammable, which is why nothing battery-powered, petroleum-based, or otherwise flammable enters a chamber. The FDA has reminded providers and patients that these safety practices exist because the hazards are real, and reputable facilities follow the national fire-protection standards written specifically for hyperbaric medicine.
Who shouldn't do HBOT — or needs extra caution
HBOT isn't for everyone. Certain lung conditions (such as some forms of untreated pneumothorax), specific medications, recent ear or chest surgery, and some acute illnesses can make treatment inadvisable or require adjustment. This is precisely why treatment should begin with a physician evaluation that screens your history — and why any facility willing to skip that step should be avoided.
What a safety-conscious patient should ask
Look for: hard-shell FDA-cleared chambers; physician supervision; staff trained and certified in hyperbaric operations; a documented safety and fire-protection protocol; and the emergency capabilities of an accredited medical facility.
